Rehabilitation Specialist - Adult CCS

JOB REQUIREMENTS: Essential Duties and Responsibilities Serve as amember of the CCS team, providing person-centered services toindividuals with mental health, substance use, and co-occurring needs,based on their strengths and recovery goals. Deliver individual skilldevelopment services such as communication, problem solving, decisionmaking, self-regulation, and conflict resolution, as outlined in theIndividual Recovery Plan. Provide training in daily living skillsincluding personal care, housing, benefits, household tasks, financialmanagement, transportation, parenting, and accessing communityresources. Use varied methods such as role modeling, mentoring,monitoring, assistance, and cuing to support individuals. Offer servicesin community, including individuals' homes and other relevantlocations, and ensure transportation for CCS-related appointments.Collaborate in Recovery Team meetings to help develop and monitorindividualized recovery plans and goals. Maintain accurate and timelydocumentation in compliance with agency policies, governmentregulations, and applicable local, state, and federal codes. Attendstaff meetings, training, and other required events, and maintainflexibility in schedule to meet program needs while following all agencypolicies and procedures. OTHER EXPERIENCE AND QUALIFICATIONS:Job Qualifications: Required Qualifications Bachelor's degree in socialwork, psychology, or a human service-related field is required. 2 yearsof experience serving people with mental health, substance use, and/orco-occurring needs preferred. Meet all WCS and contractor requirementsfor criminal and caregiver background checks, driver's record, and drugscreen. Valid driver's license, automobile, and sufficient insurance tomeet agency requirements. Knowledge, Skills, and Abilities Adhere to theagency Code of Conduct, all relevant policies, procedures, and fundingrequirements. Act ethically and maintain professional, positiverelationships with staff, individuals served, and external partners.Demonstrate cultural sensitivity and a strong commitment to theagency's mission and values. Possess knowledge and experience workingwith individuals in recovery with mental health, substance use, andco-occurring needs. Display strong organizational, assessment, andinterviewing skills; build rapport quickly and navigate difficultsituations effectively. Remain flexible, able to multitask, and use timeand resources efficiently to support high-quality CCS service delivery.Other Job Information (if applicable): PHYSICAL DEMANDS: Position ismobile with significant time spent in the community, includingtransporting individuals being served, home visits and attendingmeetings; driving throughout Milwaukee County. Ability to navigatestairs. WORK ENVIRONMENT: Work is performed primarily in the community,including people's homes and various community-based locations. Abilityto work in an office setting.
