Summary The Licensed Practical Nurse (LPN) - Primary Care reports to Nursing, Ambulatory Care Service at the G.V. (Sonny) Montgomery VA Medical Center in Jackson, MS. This position provides safe, routine care to Veterans within inpatient and outpatient units. The primary responsibility of the LPN is to provide practical nursing care to patients with a variety of physical and/or behavioral problems. Responsibilities Total Rewards of a Allied Health Professional The LPN - Primary Care will initiate appropriate actions based upon his/her observations of the patient's condition in collaboration with the Registered Nurse (RN). The duties of the LPN-Primary Care may include, but are not limited to: Recognizes the need for emergency measures when indicated, promptly seek the assistance of the RN or PROVIDER. Collects data relevant to patient's learning needs, learning preference and readiness to learn. Prevents urgent care and/or unplanned visits based on patient centered care. Articulates Patient Aligned Care Team (PACT) programs and how it is utilized. Assists in monitoring for any evidence of abuse/neglect and demonstrates knowledge of the abuse reporting process. Encourages patient to verbalize concerns and fears regarding end-of-life issues and losses. Implements interventions to maintain skin integrity and prevent skin breakdown. Adapts the environment to promote comfort and maximal functioning, self-care and independence. Maintains safe and therapeutic environment by providing one-on-one observation for behavioral health patients as directed by the Provider. Administers prescribed therapies using appropriate judgment and in and in accordance with medical center and Ambulatory Care policies and procedures, under close supervision: o Provides care to patients with various prosthetic devices o Performs phlebotomy procedures o Performs medication reconciliation o Bladder scan-pre and post void residuals Responds to verbal and non-verbal communication skills. Communicates effectively and appropriately when transferring patients to different levels of care - ISBAR handoffs. Serves as a mediator for patient, families and colleagues to improve the quality of communication and interactions in selected care settings. Supports interdisciplinary teamwork to resolve issues. Communicates information to Nurse Manager/designee in a timely manner regarding assigned patients throughout tour of duty. Documents actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, skin tests and inhalation medications as permitted by approved local facility policies and procedures. Collects and documents preventive indexes and clinic practice guidelines. Documents interfaculty consult transfers to main facility, contacts transportation with direction from Provider. Recognizes and reports to supervisor breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action. Monitors and restocks exam rooms with medical/clerical supplies. Follows reusable medical equipment (RME) guidelines to ensure appropriate cleaning and disinfecting of durable medical equipment. Work Schedule: Monday - Friday: 7:30am - 4:00pm, or 8:00am - 8:00am - 4:30pm; subject to change based on the needs of the facility. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
