This job was posted by : For moreinformation, please see:DescriptionSUMMARYThis role is responsible for performing lead case management duties as aQIDP for our DD 24-Hour CILA locations and/or ICILA locations. Maintaina caseload. Oversee and update monitoring spreadsheets of the full QIDPteam to ensure full compliance with Rule 115 and passing of BQM, BALC,surveys and audits. Integrate, coordinate, and monitor active schedules,strategies, and outcomes for individuals with disabilities. SuperviseDirect Support Professionals (DSP) assigned.ESSENTIAL DUTIESModel best practices as the QIDP to the case management team of the24-CILA and/or ICILA Residential programs.Supervise, train, oversee, and monitor case management documents andfunctions of the CRS programs to ensure full compliance with allDepartment of Human Services (DHS) rules and regulations.Train direct service staff on client strategies, outcomes, medical andbehavioral needs, and other aspects of client plete case management and medical service paperwork within Departmentof Human Services (DHS) standards and timeframes to ensure rates arealigned with client needs.Monitor and assess the use of awarded hours with the Primary QIDP,Program Managers, and Director, apply for additional support funding forclients when necessary and respond to all requests for documentationfrom DHS in a timely manner.Maintain program compliance with funding body requirements.Transfer monthly billing for the CRS programs and collaborate withProgram Managers and Director to meet financial goals established inbudget related to 31C, 31 U, 31 V 53D, and Home-Based plete quality assurance duties which include being knowledgeable ofDHS Rule 119, maintaining client records, and all medical documentation.Participate in necessary training, meetings, and committees as needed ordirected.Collaborate with the Board-Certified Behavioral Analyst (BCBA), SpecialSupport Team (SST), and other team members to improve services for theclients and promote continuity across environments.Serve as an active member of the Community Support Team (CST) forclients receiving services across the 24Hour CILAand ICILA Residentialprograms.Ensures timely completion of all client paperwork, includingdocumentation data on all goal objectives and support developed withclient; client rights and releases are signed annually and are current,as required by Agency and DHS Policy.Assist in coordinating client referrals and intake, admission,transfers, and discharge in conjunction with the Program Manager, fellowQIDP, and Director.Collaborate with Individual Service Coordinator (ISC) and Home-BasedFacilitator or Service Direct Administrator (SDA) ensuring ICAP scores,Assessments, Q-Notes, Discovery Tool, Personal Plan, ImplementationStrategies, and all relevant updates are completed annually, within DHSrequired guidelines and time frames.Provide direct support in problem or crisis situations as required.Assist in collaborating with the Accounting Department, responsible forfollow -up on rejected billing.Develop and maintain a cooperative relationship with residentialfacilities, families, and other community agencies/members to facilitatereferrals, coordinate implementation strategy plans and improve programservices.Respond to medical and behavior crisis in accordance with agency policyand procedures. This includes related paperwork.Ensures accurate and timely completion of documentation and paperworkwithin Anixter Centers Electronic Health Record (EHR).Confirm daily and weekly data is being tracked for each client regardingoutcomes and behavior.Ensure completion of assigned OJT's and new DSP training within the120-day period as required by D S in conjunction with Program Managers.Establishes and maintains relationships with the individuals CommunitySupport Team (CST).Stays updated on program changes per governing body for effectiveexternal and internal communications. Complete quality assurance dutieson all files, which includes being knowledgeable of DHS Rules plete and maintain program records, schedules, and reports.Provide program coverage in case of absences, vacancies, or otheremergency situations within CRS programs. Assume direct responsibilitieswhen program coverage makes it necessary including driving all agencyvehicles and transporting clients.Ensures adherence to all mandated reportering, OIG guidelines, and plete an incident report when an incident arises. Collaborate withthe Primary QIDP, Program Managers, and Director to ensure that allincident reporting is completed accurately and in a timely manner.Review and follow-up on incident reports and safety within a 4-hourtimeframe and report critical incidents through DHS Critical IncidentReporting Analysis System (CIRAS).Responsible for ensuring timely communications are sent to the CommunitySupport Team (CST) pertai
