FTE: 0.500000 Standard Hours: 20.00 Shift: Shift 1 Shift Details:Hours vary between 8-4:30, days worked vary based off clinic needs.Holidays: None Weekends: None Job Summary: The Interventional RadiologyMSK RN Coordinator is responsible for the coordination of care ofpatients within the Musculoskeletal (MSK) current and future programs.Current programs include the Vertebral Compression Fracture Pathway andSkeletal Metastasis Program. The coordinator is responsible forassessing and triaging new patient referrals, coordinating patientclinic visits and follow up, reviewing clinic processes, developing andimplementing clinical workflows, working collaboratively withinterprofessional multidisciplinary teams to support MSK IR researchprojects, developing and implementing patient and staff educationresources, and developing quality initiatives to promote the safe andefficient care of the MSK patient population served within the IRclinic. The Clinical Coordinator - Ambulatory Clinics supports aspecialty population and/or group of specialty patients by coordinatingpatient care at the local level, across sites and settings, and/for theduration of the care continuum. The Clinical Coordinator facilitates thedevelopment and provisioning of clinical resources; assists withevidence-based practice & quality improvement initatives andfacilitating specialty training and orientation. Required skills includecritical thinking, excellent verbal and written communication, theability to work autonomously/be self-directed, collaborating withothers, acting as a change agent, and ability to teach peers. TheClinical Coordinator is able to complete all duties of the Staff RN rolein their designated care area. In addition, this nurse workscollaboratively as an active member of the interprofessional team toprovide therapeutic patient and family centered care. This rolesystematically and continuously collects and assesses data related topatient health status to develop, execute and evaluate the plan of care.The nurse utilizes evidence to drive clinical decisions and practicerelated to individual and population health. The nurse establishes goalsand strategies for meeting care coordination needs across settings withthe patient, family, and/or other care providers. The nurse considersfactors related to safety, effectiveness and fiscal accountability inplanning and delivering care. The nurse delegates and supervises care inaccordance with the WI Nurse Practice. This role participates incontinuous quality improvement initiatives and assumes responsibilityfor their professional development. EXPERIENCE DESCRIPTION: A minimum of3 years as a nurse in a healthcare setting required. Candidates with twoyears as a nurse in the listed specialty (i.e. - dermatology,cardiology, etc.) may be substituted for three total years of nursingexperience. Experience in the specialty strongly preferred. EDUCATIONDESCRIPTION: Nursing licensure required. Bachelor's Degree in Nursing,preferred. LICENSURE DESCRIPTION: Requires current state of WisconsinRegistered Nurse License or Multi-state Nursing License from aparticipating state in the NLC (Nurse Licensure Compact). American HeartAssociation (AHA) Basic Life Support for the Healthcare Provider (BLS)or an AHA approved equivalent is required within 90 days of hire.
