As a Hospice Home Health Aide, you will provide essential, hands-on care, ensuring dignity and well-being for our patients. Under the guidance of a Registered Nurse, your daily impact will include: + Personal Care & Comfort: Assisting with personal hygiene, mobility (ambulation and transfers), and promoting overall patient well-being. + Patient Monitoring: Diligently reporting any changes in condition to the nursing team. + Supportive Environment: Performing light housekeeping, preparing meals, and ensuring a safe, healthful, and comforting home environment. Job Requirements + High School Diploma or equivalent required. + Certified Nurse Assistant (CNA) in the state of Minnesota (MN) required. + Basic Life Support (BLS) required. + Must have a valid Driveru2019s License. + Must have current vehicle insurance.
