Please read below and apply with an updated resume for consideration: Pay: $51to $55.75 per hour based on experience Type: Four-month contract Schedule: 5 days per week 8:00AM u2013 4:30PM Job Description: The Home Health Field Registered Nurse (RN) plays a critical role in delivering high-quality, patient-centered care within the comfort of patients' homes. This position involves comprehensive assessment, planning, implementation, and evaluation of nursing care tailored to individual patient needs, often managing complex medical conditions. The RN collaborates closely with interdisciplinary teams, patients, and their families to promote health, prevent complications, and support recovery or chronic disease management. By providing education, emotional support, and clinical expertise, the RN empowers patients to achieve optimal health outcomes and maintain independence. Ultimately, this role ensures continuity of care outside traditional clinical settings, improving patient satisfaction and quality of life. Primary Responsibilities: u00b7 Initial and ongoing comprehensive assessments and treatment on the patients physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs. u00b7 Apply specific criteria for admission and re-certification to home health care to establish appropriate levels of care and the patientu2019s eligibility. u00b7 Implement, develop, and document the plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary. u00b7 Consult with and educate the patient and family regarding self-care techniques, nutrition, and dietary needs. u00b7 Determine scope and frequency of services needed based on acuity and patient/family needs. Assess the ability of the caregiver to meet the patientsu2019 immediate needs upon admission and throughout care. u00b7 Initiate appropriate preventative and rehabilitative nursing procedures. u00b7 Apply concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease. u00b7 Provide clinical directions to the Home Health Aide and LPN/LVN to ensure quality and continuity of service provided. u00b7 Prepare clinical and progress notes that demonstrate progress toward established goals. u00b7 Inform physicians and other personnel of changes in the patientsu2019 needs and outcomes of intervention, while evaluating patient and family response to care. u00b7 Monitor assigned cases to ensure compliance with requirements of third-party payor. u00b7 Provide effective communication to patients, family, team members, and other health care professionals.
