Summary:Responsible for clinical review and authorization of post acute care services including Skilled Nursing Facilities (SNF), Inpatient Rehabilitation Facilities (IRF), Long Term Acute Care Hospitals (LTACH) and other services as appropriate. Evaluates medical necessity, level of care appropriateness, discharge readiness, and care transitions using evidence-based guidelines and health plan policies. Supports members in achieving safe, timely, and cost effective transitions across care settings.Acts as a bridge between insured members, healthcare providers, and insurance providers, to optimize quality outcomes, member experience and prevent hospital readmissions. Minimum Qualifications: 1.Formal Education Required:a. Bachelor’s Degree or equivalent combination of education and/or experienceb. Registered Nurse (RN) state license in good standing.2. Experience & Training Required:a. Three (3) years’ experience to include previous industry in post-acute settings (SNF, acute rehabilitation, or LTACH), care coordination and/or homecare or community agency experience.b. Experience performing Utilization Management, Prior Authorization, or Concurrent Review c. Proficiency using clinical criteria such as InterQual? or MCG?.d. Strong assessment, communication, and clinical documentation skills.e. Prior health plan experience strongly desiredf. Ability to work independently, manage multiple cases, and meet regulatory turnaround times.g. CCM/ACM certification or equivalent desired3. Other Skills, Competencies and Qualifications:a. Skill in keyboarding acquired through previous experience. b. Excellent ability to communicate instructional and operational information verbally and in written form that may include correspondence, reports, instructional materials, system designs. c. Ability to provide instruction and guidance required to supervise day-to-day departmental operations; ability to interface with internal staff, mid-upper management professionals, and general public.d. Skill in collecting, analyzing data from diverse sources; making recommendations and/or conclusions by evaluating the medical necessity of requested post-acute services (e.g., skilled nursing facility, LTAC, acute inpatient rehab, other post-acute services) against evidence-based criteria like InterQual; developing financial, data processing technical reports, procedures, systems that usually affect one department. e. Detail oriented with proofreading skills for completed work to find and correct errors. f. Demonstrate knowledge and skills necessary to provide care appropriate to the age of the members serviced. Takes ownership in identifying and obtaining education and training needed to meet needs of the members.g. Work cooperatively with team members to accomplish organizational mission and goals.h. Excellent time management skills to organize and manage time to accurately complete tasks within designated time frames in fast paced environment.i. Maintain current knowledge of and comply with regulatory and company policies & procedures.j. Maintain confidentiality of member health and business information.k. Flexible: ability to adjust work hours to meet business demands.l. Frequent overtime work throughout the year, with heavy overtime during peak periods. Occasional travel may require use of personal auto to visit a member at an area facility. 4. Level of Physical Demands:a. Sit and/or stand for prolonged periods of time.b. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay.
