What Youu2019ll Do As a Home Health RN, youu2019ll be the heart of patient careu2014bringing clinical excellence directly into patientsu2019 homes. + Manage a dynamic caseload of patients in your local community + Perform comprehensive assessments and develop individualized care plans + Provide hands-on skilled nursing care and patient education + Monitor patient progress and adjust care plans as needed + Collaborate with physicians, therapists, and interdisciplinary teams + Be a trusted partner for patients and families throughout their care journey -------------------------------------------------------------------------------------------------------------------------- ? What Youu2019ll Need + Active, unrestricted RN license (Connecticut) + At least 1 year of clinical nursing experience + Ability to travel locally (youu2019ll spend time in the fieldu2014this is not a desk job) + Valid driveru2019s license, reliable vehicle, and current auto insurance + Ability to safely assist patients (lifting, positioning, etc.) Preferred (but not required): + Home Health or Hospice experience + Familiarity with HomeCare HomeBase + Associateu2019s or Bacheloru2019s Degree in Nursing -------------------------------------------------------------------------------------------------------------------------- u2728 Why Elara Caring?
