Summary The Registered Nurse Oncology/Hematology Registered Nurse (RN) supports the VISN 6 Cancer Clinical Research Network (CCRN) site.The RN possesses foundational expertise, demonstrating introductory proficiency in delivering both direct and indirect nursing care to Veterans with cancer patients and their families. The RN collaborates with the interdisciplinary team to achieve desired patient outcomes. Responsibilities The RN functions as a nurse navigator and care coordinator for Veterans with cancer across VISN 6. The RN provides education to Veterans, and connects Veterans with essential services such as social work, nutrition, psychology, chaplaincy, and physical/occupational therapy at their local VA facility or other VA facilities in VISN 6. The RN works with other team members to support second opinion consults, multi-disciplinary conference reviews, and interfacility referrals for specialized care across VISN 6. This also includes VA cancer pathway documentation in VISN 6, by identifying cases, alerting providers to document, and completing documentation when necessary. The RN works with a Veterans advisory committee, comprised of cancer survivors and caregivers, who review and provide input on CCRN initiatives and programs. Duties include, but are not limited to: Completes assessments and completes documentation in the electronic health record (EHR) in accordance to established policies and procedures. Triages patient problems and assists in the evaluation of symptoms and initiate interventions in collaboration with the interdisciplinary team. Monitors, tracks/tends, and reports ongoing care needs and outcomes to the interdisciplinary team. Direct the provision of nursing education, orientation, competencies, and providing quality improvement and outcome utilization. Conducts in-service education based on insights gained from oncology professional development activities and acquiring specialized skills in oncology-specific areas.
