Case Management Analyst u2013 LTSS (Virginia u2013 Field Based) _Make a difference where it matters most._ Aetnau2019s Longu2011Term Services and Supports (LTSS) program is looking for a passionate, memberu2011focused Case Management Analyst to support individuals and families across the Northern Virginia region. This is a fieldu2011based role where youu2019ll spend meaningful time faceu2011tou2011face with membersu2014listening, advocating, and helping them navigate complex health and social needs. If youu2019re driven by purpose, enjoy working independently in the community, and value building strong connections, this role offers the opportunity to truly impact lives. _What Youu2019ll Do_ + Build trusted relationships with members through inu2011person visits (approximately 70% travel). + Conduct comprehensive assessments using clinical judgment and critical thinking to understand medical, behavioral, and social needs. + Develop and implement individualized care plans that promote health, independence, and quality of life. + Collaborate closely with providers, community resources, and interdisciplinary teams to ensure coordinated, highu2011quality care. + Connect members to services, supports, and resources that align with their goals. + Ensure care delivery meets regulatory requirements and Aetna quality standards. + Advocate for medical appropriateness while supporting positive member engagement and outcomes. _Required Qualifications_ + 2+ years of experience in behavioral health, social services, or a related field aligned with program focus. + Residency in one of the following Virginia areas: Alexandria, Arlington, Vienna, McLean, Woodbridge, and/or Springfield. + Reliable transportation and the ability to travel up to 70% of the time (mileage reimbursed per company policy). _Preferred Qualifications_ + Experience in case management or discharge planning. + Managed Care experience. _Education_ + Bacheloru2019s degree or nonu2011licensed masteru2019s degree required. + Degree in behavioral health or human services strongly preferred. Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $21.10 - $40.90 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
