Infectious Disease - Ambulatory Care The Ambulatory Care Clinical Pharmacist will optimize drug therapy outcomes by providing high quality, cost-effective health care; influencing prescribing behaviors; identifying, preventing and resolving drug-related problems; and educating health care personnel regarding drug therapy. Participate in clinical programs to improve patient care, including but not limited to participating in in-person or telephonic pharmaceutical care clinics, therapeutic drug monitoring programs, participate in consult agreements, and programs to support state-wide initiatives. Current practice for this role involves but is not limited to: + Managing OPAT patients; responsibilities include completing a referral note within 48 hours of hospital discharge, monitoring weekly (or 2x weekly) labs, managing side effects, aiding in transitions of care concerns, following clinical progress, and determining plan of care at end of therapy + Rounds with each RN/MD team to review OPAT list + Aid in HIV and PrEP counseling and medication management (including prior auths and insurance concerns where needed) + Aid in TDM for long term antifungals + Organize and facilitate PGY1 ID rotation for The Jewish Hospital u2013 Mercy Healthu2019s Residency Program + Answer any drug information questions from MDs, RNs, inpatient pharmacists etc. This role delivers patient care utilizing the nursing process through data collection and assessment, identifies and determines the priority of patientu2019s problems/needs, and evaluates the process and outcome of nursing care. They coordinate, provide, and direct patient teaching activities and coordinate the care provided by health team members. The Clinical Pharmacy Specialist participates in clinical programs and initiatives >90% of the time with a consistency in schedule. The Clinical Pharmacy Specialist has received specialized training in their dedicated area of practice and ensures safe, rational, and cost-effective therapy in their respective practice area. The Clinical Pharmacy Specialist will act in compliance with all applicable rules, regulations, and standards as promulgated by Federal, State, and accrediting agencies or regulating bodies. The Clinical Pharmacy Specialist maintains established departmental policies and procedures while promoting department objectives. Essential Functions: u00b7 In collaboration with the interdisciplinary care team, conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment and evaluates treatment effectiveness u00b7 Administers medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures u00b7 Serves as point of contact for patients' care coordination throughout hospital departments. u00b7 Acts as a patient safety advocate by participating in ongoing quality improvement in the department. Incorporates the professional practice model into care delivery. u00b7 Provide comprehensive pharmaceutical care to a focused population and serve as a subject matter expert in area of specialty. u00b7 Patient focused activities may include but are not limited to in-person/telephonic pharmaceutical care clinics or patient care areas, therapeutic drug monitoring programs, consult agreements and programs to support ministry and/or state-wide initiatives This document is not an exhaustive list of all responsibilities, skills, duties, requirements, or working conditions associated with the job.
