Collaborates with care management, physicians, and clinicians to develops discharge plans requiring post-acute services. Ensures seamless transitions across the continuum of care. Educates patients and families about post-acute care services and expectations. Assists in assessing patients for post-acute care and coordinates the clinical transition to home health, hospice, or other appropriate settings. Maintains comprehensive knowledge of managed care and community resources. Completes and submits all documentation in a timely manner according to department policy. Participates in multidisciplinary rounds, care conferences, and coordination with case management. Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers. Coordinates efforts for treatment and promotes quality improvement and educational efforts. Provides referrals and supports for follow-up care for patients who are uninsured or underinsured.Other duties as assigned.Knowledge, Skills, and Abilities: • Competent with Microsoft Office [Required] • Possess strong organizational skills with the ability to prioritize, manage and complete multiple tasks and projects within specified timeframes. [Required] • Age specific care for program of focus/clinical area of specialization. [Required] • Teaching and counseling patients, families and staff as needed. [Required] • Strong communication skills.
