Summary The Neurology Clinical Pharmacist Practitioner functions at the highest level of clinical practice, working independently with prescribing privileges under a scope of practice as defined by the medical center to directly care for patients served by the Neurology service and to other areas of the medical center as assigned. Responsibilities VA Careers - Pharmacy: Total Rewards of a Allied Health Professional A. Coordinates all clinical pharmacy functions for assigned practice area, helping to achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams. Attends and participates in multidisciplinary rounds, team meetings, conferences, and other meetings as required. B. Designs, implements, assesses, monitors and documents therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment for patients consulted to neurology service and other clinics as designated by supervisor. C. Works independently and effectively to evaluate prescriptions and/or provider orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; contacts physicians and makes authorized changes in orders or prescriptions as required. D. Functions independently in day-to-day clinical and administrative duties associated with the position, consulting with higher level supervisors only when required. In general, uses a high degree of appropriate professional judgment to solve complex problems which routinely arise throughout the work day. When required, communicates any problems to the medical staff and/or other health care professionals, and offers alternatives or solutions to the problems. E. Demonstrates recognized expertise in the assigned area of practice for the purpose of management of several disease states, including-but not limited to-headache, epilepsy, movement disorders, amyotrophic lateral sclerosis (ALS), multiple sclerosis (MS), stroke. F. Monitors and assesses the outcome of drug therapies with advanced skill, including physical assessment and interpretation of laboratory and other diagnostic procedures. Works closely with neurology team to monitor patient response to medications - including efficacy, safety, and outcome(s). G. Demonstrates advanced knowledge of pharmaceutics, pharmacodynamics and pharmacotherapeutics as determined by peer review. Reviews, analyzes, and applies literature to specific patient situations. H. Documents interventions and recommendations via CPRS progress notes and consults, demonstrating the ability to communicate orally and in writing to persuade and influence clinical and management decisions. Written documentation of all patient interactions is required. I. Evaluates the need for and provides verbal and written medication information to patients or families concerning their medications and disease states. J. Assists the area of practice in meeting all of The Joint Commission medication management standards by having an expert understanding of regulatory and quality standards for the area of practice. K. Evaluates non-formulary and restricted medication requests from assigned providers. L. Supervises pharmacy trainees and residents assigned to participate in the program. M. Participates in other clinical pharmacy activities as assigned by supervisor. Work Schedule: Monday-Friday 8:00am-4:30pm Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Requirements Conditions of Employment You must be a U.S. Citizen to apply for this job.
