Job Description

Job Purpose:Provide medical nutrition therapy and diabetes education to improve outcomes and quality of life.Essential Duties and ResponsibilitiesProvides individual medical nutrition therapy and individual and group diabetes education to meet patient goals, through in person and/or telehealth sessionsAssesses, evaluates, interprets, monitors, and documents nutritional status and needs of patients; uses established standards of care and evidence-based practice guidelinesCounsels and provides tailored nutrition education to patients and their familiesUses age-specific, disease-specific and culturally competent educational materials and counseling techniques to meet patient needsDocuments in electronic medical record to monitor patient progressCommunicates and collaborates with patient’s primary care provider, as well as other specialty services to coordinate careAttend all mandatory trainings and department meetings, readily bringing own ideas and thoughts into the discussionDevelops, reviews, updates, and implements educational materials to meet the needs of patients and providersProvide in-service training or presentations for other company providers and specialists, as deemed necessaryContribute blog articles and recipes to company websiteContribute case studies and data on patient progress as part of outcomes collection and research activitiesContinually study emerging trends and new research related to the practice of dietetics and facilitate discussion and findings with the nutrition teamTrain patients on continuing glucose monitoring devices upon completion of training programMaintain license through NC Board of Nutrition and Dietetics and credentials with the Commission on Dietetic RegistrationComplete at least 15 diabetes-related continuing education credits annually to maintain instructor status in Avance Care accredited diabetes program, in addition to completing continuing education to maintain RDN credentialsAdditional Duties and ResponsibilitiesAccomplishes all tasks as assigned or as they become necessaryQualificationsExperience, Education and Licensure:Registered Dietitian through the Commission on Dietetic Registration Licensed Dietitian through the North Carolina Board of Nutrition and Dietetics Master’s Degree, preferredAt least 1 year of clinical nutrition experience, highly preferredDiabetes education experience, preferredKnowledge, Skills, and Abilities:Strong written and verbal communicationStrong computer skillsStrong attention to detailTeam playerEfficient time management and workload prioritizationLanguage Skills:Ability to communicate well in writing and in person. Ability to read and interpret common medical terminology.Expectation of EmployeeAdheres to Avance Care’s Policy and ProceduresActs as a role model within the clinicPerforms administrative and clinical duties as workload necessitatesMaintains company and patient confidentiality both on and off work premisesMaintains a positive, respectful attitude, high team spirit and moraleUses tact in all personal interactionsConsistently reports to work on time for scheduled shift, prepared to perform duties of position
