The Social Services Specialist responsibilities include: + Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community + The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning + They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs + Possess a clear understanding of discharge and transition planning and linkage with community resources + Demonstrates skills in planning, organizing, and managing multiple functions and complex processes Position details: + Location: North Bay Hospital - New Port Richey, FL + Status: PRN, As Needed + Schedule: Days Vary, 8:30 AM - 5PM + Weekend requirement: Occasional + On Call: No Certifications and Licensures: + Preferred ACM (Case Management) or + Preferred CCM (Case Manager) Education: + Required Masters in Social Work, Sociology, or Related Field + Strongly Preferred Masters Degree in Social Work Experience: + Required 6 months Internship or + Required 1 year Social Work in an acute care or hospital setting Benefits: + Benefits (Health, Dental, Vision) + Paid time off + Tuition reimbursement + 401k match and additional yearly contribution + Yearly performance appraisals and team award bonus + Community discounts and more Equal Opportunity Employer Veterans/Disabled Equal Opportunity Employer Veterans/Disabled Position Social Services Specialist II Location New Port Richey:Morton Plant North Bay Clinical PRN Req ID 149026
