When you thrive, so does the community of carewe're buildingtogether.Offer Based on Years of ExperienceWhat You Need to KnowBe the Best Hospice Nurse Practitioner You Can BeIf you meet these qualifications, we would love to meet you:- Graduated from an approved school of professional nursing - Masters degree- Current and unecumbered advanced practice nurse license in the state(s) practicing- One year of experience as an NP- Certified by a nationally recognized advanced nurse practice organization- Possess and maintain valid CPR certification while employed in a clinical role (preferred)- Must be a licensed driver who can travel to all business locations- Meet the regulations and requirements of the state(s) in which the program provides servicesResponsibilities:As a Hospice Nurse Practitioner, you will:- Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family- Monitor patient progress; update the Team Director and team members; consult regarding discharge planning and patient/family teaching- Maintain continuity of care in coordinating with the care team and case manager for patients with ongoing needs- Prescribe medication as permitted by the state and/or the state Board of NursingPre-Hospice
