Job Summary and Qualifications AsaNurse Navigator, you willfunction as a member of the multidisciplinary team andserveas an advocate and educator for patients with all types of medical conditions.You willbe responsible forsupportingpatients through complex healthcare environments for the optimization of care and outcomes. You willfacilitatepatient follow-up through coaching, information transfer, and communicationtoimprove continuity of care, enhance the medical management of complex diseases, and streamline the intervention process. You will alsoguide patients and families through the healthcare system byassistingwith access issuestoexpeditethetimeliness of care and treatment.You willachieve this goal by building relationships with patients and physicians, coordinating the defined plan of care,assistingwith appointments,assistingin resolvingbarriers, providing education, and connecting with community resources.You willalso assume responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for patients, including support with interdisciplinary and cross-facility coverage and collaboration.Youareresponsible fortimelydata collection that promotes tracking of the navigation program performance in line with HCA Healthcare, division,and facility goals (growth, quality, practice guidelines,etc). What you will do in this role: Serves as a patient advocate Assesses the patient’s medical and psychosocial needs astheypertainto their primary medical or surgical condition for navigation Usesappropriate toolstoidentifypatient needs and barriers to care and provides access to potential resolutions Identifieshealth disparities andassistsintheremoval of these disparities Providesappropriate education, outreach, and support to patients and families. Ensures the patient is empowered to manage his/her healthcare needs Provideshealthcare system guidance and promotes adherence to recommended treatment throughout the care pathway Supports providers byassistingpatients and/or families in understanding their diagnosis, treatment options, and the resources available, including education, clinical research studies (as applicable), andtechnologicaladvances Provides education/connection to community and external resources, such as access to supportive care, financial support, and return to work Streamlinesprocessfor patients byassistingwith appointment scheduling in a compliant manner Ensures the organization of appointments and explains the sequence of treatmentsEnsures smooth transitions between care modalities, facilities, and providers Facilitates patient movement through theappropriate clinicalpathway and collaborates with physicians to ensure patient compliance Reinforces patient-specific teaching as defined by the patient’s provider Conducts follow-up conversations with patients and communicates any concerns, changes, or social needs to theappropriate physicianor other care provider Attends andparticipatesin Multidisciplinary Meetings (MDM) and/or other care planning conferences, as required bythenavigation program Ensuresappropriate patientdata is available, including assessment and documentation Educates the patient and familyregardingplanned procedures, medications,and treatments What you will need in this role: 1-3 years of experience in Nursing - Required 3+ years of experience as a Nurse Navigator and/or patient educator in service line - Preferred Education Associate’s Degree in NursingrequiredBachelor’s Degree in Nursingpreferred Licenses and Certifications Basic Cardiac Life Support (preferred if remote position) - RequiredRegistered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices,in accordance withlaw and regulation. - Required Benefits St. David's North Austin Medical Center, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
