Click here: MercyOne Iowa Heart Center Des Moines MercyOne ( Join the MercyOne Family We are looking to hire an RN- Navigator: Heart Failure/ Cardiac for our MercyOne Iowa Heart Center Des Moines. As a RN Navigator: Heart Failure/Cardiac at MercyOne, you will assists with the development and implementation of a Center of Excellence for Heart Failure Management Program using evidence based guidelines for recommends care. This position will focus on strategies to promote and navigate care transitions. The navigator provides support, education and coordination of care for the patient and their family. Guides patients through the complexities of the health care system. Acts as an advocate for the patient and family. Communicates across health care areas to ensure that systems integrate efficiently in order to best serve patients, staff, physicians and community. The Heart Failure Navigator is experienced in cardiovascular nursing and the care of heart failure patients; and is able to integrate a working knowledge of diagnoses and procedures to assist patients in the outpatient setting, through hospital stay and rehabilitation, and post-acute care. Coordinates patient care with other multidisciplinary team members. This position reports to the Director of Cardiac Specialty Services in the Cardiovascular Service Line. Schedule: + Full time 40 hours a week + 8:00-4:30 + No Weekends or Holidays General Requirements: + Coordination of Care: Coordinated care for all appropriate patients throughout the continuum of care from diagnosis through rehabilitation. + Reconciles daily census for identifying heart failure patients in hospital. + Daily rounds on heart failure patients. + Participates in education through patient assessment, needs assessment, and initiation of discharge planning. + Collaborates with the patientu2019s physician(s), primary nurse, coordinator, ancillary staff and extended care provided throughout the patients hospital stay to coordinate patientu2019s plan of care and final discharge plan. + Participates in data collection and other quality improvement initiatives during hospitalization as directed. + Is an advocate for the patient and/or family. + Provides follow up support post discharge at specified intervals. + Follow-up phone calls as required. + Participates in data collection post operatively and post discharge as directed. + Works collaboratively with heat failure team and alternatively as heart failure nurse as needed (office or hospital). + Patient Education + Provides patient/family education as directed. + Provides educational materials via other routes when necessary. + Assists with reviewing, developing and updating materials to facilitate the education of patients and coaches. + Communication/Consultation: + Collaborates with physicians, offices, staff, and other multidisciplinary team members to facilitate the care of the patient related to continuity of care and coordination of care and education. + Works closely with heart failure team and acts as an alternate to nurse when appropriate. + Works cooperatively with extended care providers (home health, skilled care, rehabilitation and outpatient physical therapy) to ensure quality patient outcomes. + Participates in Joint Bundle committees as directed. Education: + Registered nurse with valid license in State of Iowa required. + Bacheloru2019s degree in healthcare field or actively pursuing, (BSN preferred). + Two (2) years of related work experience required, - 4 years preferred. + Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire. + Excellent communication skills. + Presentation skills with small and large groups. + Valid BLS (Basic Life Support) certification required within 3 months of hire. Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers.
