Minimum Requirements Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.Active Georgia Physical Therapy license required to start the assignment. Compact licensees accepted.Current BLS certification from AHA or ARC may be required.Minimum 2 years of home health experience preferred. This includes experience in home health documentation and care coordination.Experience with HomeCare Homebase preferred.Ability to perform comprehensive PT evaluations, develop measurable goals and treatment plans, and document per company, funding source, and regulatory requirements.Experience supervising PT Assistants and PT Aides in accordance with state practice act preferred. Assignment Details Facility Type: Home Health.Shift Name: Days.Shift Details: Days 0800-1700, Monday through Friday.Assignment Type: Travel contract to contract-to-hire opportunity.Counties Covered: Lowndes, Echols, Brooks, Cook, Berrien, and Lanier (travel between patient homes within these counties expected). Interstate access via I-75 provides regional connectivity and drives to Tallahassee (about 90 miles) or Jacksonville (about 140 miles) for cultural and airport options. Impacting Patient Care Perform physical therapy evaluations and interventions in patient homes based on physician orders and established protocols. Communicate patient status and progress to physicians, families, and the care team. Recommend and assist with adaptive equipment. Maintain timely clinical and billing documentation.
