Summary The Nurse Coordinator provides comprehensive case management and care coordination for patients across Vascular Surgery and multiple Medicine-Surgery specialties at VA Puget Sound. This role supports complex patient needs, facilitates pre- and post-operative care, manages patient communication, and ensures timely follow-up of specialty testing and imaging. Responsibilities Duties include but are not limited to: Serves as the Primary Nurse Coordinator for the Plastic Surgery service, with additional responsibility to offer float coverage to other specialty areas as needed. Delivers Medicine Specialty coverage across Cardiology, Endocrinology, Infectious Disease, Neurology, Rheumatology, Pulmonary, and Sleep Medicine. Supports Surgery Specialty coverage across Cardiothoracic Surgery, General Surgery, Gynecology, Neurosurgery, Ophthalmology, Orthopedics, Otolaryngology (ENT), Plastics, Podiatry, Urology, and Vascular Surgery. Cross-trains across multiple specialty clinics to ensure adequate coverage in areas with staffing gaps, extended leave, or vacancies. Performs case management for Medicine and Surgery patients referred from VISN 20, ensuring coordinated and patient-centered care. Assists with continuity of care for Veterans with complex or chronic medical conditions requiring long-term coordination. Facilitates surgical and procedural care, including pre- and post-operative education, travel and lodging arrangements, social work collaboration, and follow-up needs. Delivers effective patient and family education related to disease processes, medications, diagnostic procedures, and treatment plans. Manages communication with patients through phone calls, secure messaging, and CPRS alerts to ensure timely and accurate information exchange. Collaborates effectively with interdisciplinary team members to promote safe, efficient, and high-quality patient care. Ensures specialty-ordered testing and imaging are appropriately documented, tracked, and communicated, with timely notification of results to patients as required by VA policy.
