Care Coordination Assistant

The Care Coordination Assistant Responsibilities Include: + Entering new referrals and updates all existing patient information in corporate IS systems including demographics, insurance, and benefit information. + Verifying insurance and benefits for all Medicare, Managed Care and Private Fee for Service Plans. Securing authorization for services provided by BayCare HomeCare from the payers. + Selecting the correct payer codes to provide accurate intake information. + Understanding medical criteria for securing all required information from the referral source. + Entering the appropriate financial information including benefits, deductibles, co-pays, coinsurance, out-of-pocket, another related costs into the EMR system. + Timely requesting and communicating of authorizations to payer and documentation in EMR. + The Care Coordination Assistant will work under the direction of the Care Transition Coordinator or RN Case Manager to establish and maintain a positive working relationship with patients, payors and other customers. Position details: + Location: Hybrid - Largo, FL + Status: PRN - As Needed + Schedule: 8:00 a.m. - 5:00 p.m., Monday u2013 Friday + Weekend requirement: Occasional + On Call - No Education: + Required High School Graduate or Equivalent GED + Preferred Technical Diploma u2013 Medical Specialization Experience: + Required 1 year Customer Service + Required 1 year Insurance Verification + Required 1-year Medical Related field Ready to Make a Difference?
