Assignment Details Primary specialty: Physical Therapist.Facility type: Home Health.Shift: Days.Shift hours: Monday-Friday 0800-1600 (client notes indicate Monday-Friday 0800-1630 EST schedule and Days 0800-1600).Weekend/on-call requirements: One weekend per month and one weekday on-call per week.Coverage areas: Madison, Montgomery, Clark, Powell counties.Typical responsibilities: SOC, evaluations, recertifications, ROC, agency discharges within scope of practice.Mileage reimbursed between visits from first patient to last patient.Floating to other facilities is not indicated.
