Job DescriptionBusiness OverviewAs part of the bold vision to deliver the "Next Generation" of managed care in Ohio Medicaid, Ohio RISE will help children and their families who are experiencing barriers achieve positive health outcomes and high quality of life indicators by developing person-centered and strengths based innovative solutions and creating partnerships among MCOs, vendors, and ODM to support specialization in addressing critical needs. The OhioRISE Program is designed to provide comprehensive and highly coordinated behavioral health services for children with serious/complex behavioral health needs involved in, or at risk for involvement in, multiple child-serving systems.This is a full-time teleworker position in Ohio, with regional travel required. Using lived experience with mental health and/or substance use challenges and having achieved recovery, Peer Support Specialists guide, coach, and mentor others who are working towards recovery. They will use lived experience of recovery to help the Aetna team understand best how to coordinate care for members and develop strengths-based action plans for members with whom they are providing support. Share coping strategies, wellness tools and other resources with care managers, coordinators and the members served to best meet the needs of the member where they are at in that point in time.Fundamental Components: Regularly attend case rounds and assist the team with identifying members who might benefit from Peer Support Services. Orient the care management team as to how peer support services can help members in achieving member recovery goals, reinforcing the care plan, maintaining community tenure, adjusting tasks to fit with the member's culture, language, and religion, improving access to care, identifying health care needs, and reducing unnecessary hospitalizations or emergency services. Collaborate with healthcare professionals and providers on the principles of recovery, and opportunities for improving and understanding the social determinants of health that may be impacting member's health and treatment plan through various communication channels. Collaborate with care managers and coordinators in helping members to identify socio-economic issues that affect their overall health and develop health/social management plans and goals. Identify gaps and opportunities to strengthen systems of care with the member and within the member's community and increase utilization of community services through demonstrating for care coordinators and members how to schedule appointments with social services agencies, (including transportation vendors), and to complete applications for eligible programs. May attend medical, behavior health or community appointments with members when appropriate and advocate for getting the best possible care. Assist members in setting and attaining person-centered short and long-term goals. Suggest approaches that encourage members to participate in wellness and recovery activities at a pace that is comfortable for them and supports the individual in identifying a recovery pathway that meets their needs. Develop a person-centered wellness plan in collaboration with members to aid in helping them remain in the community. Utilize lived experience in working with members to demonstrate that recovery is possible. Document activities within tracking portals. Provide education to health plan employees, as necessary, on what are Peer Support Specialist services. Meet with members face-to-face in a setting of their choice to develop a strengths-based plan of action and identify their long-term goals for care. Travel, when approved, will encompass at least 50% of individual's time regarding meeting with members in the community. Peer Support Specialist employed must have knowledge of local community and supports and services available in the community.Required Qualifications This is a full-time teleworker position regionally based in Ohio, with regional travel required. 2-3 years' experience in community/outpatient setting working with members in a mental health or substance use setting is required. If not certified, completion of a Certified Peer Support Training Program is required within 90 days from start date. Must be a recognized training program. Exceptional communication, organizational and interpersonal skills, with attention to detail required. Experience using a computer and strong keyboard navigation skills including the use of MS Office Suite, Word, and Excel applications. Ability to work effectively with a wide range of constituencies in a diverse community. Ability to travel 50% of the time within the assigned region, to multiple office and home locations with personal vehicle. Must possess reliable transportation, valid/active driver's license, and proof of insurance. Ability to educate health care providers as needed to adjust interventions based on member's cultural, ethnic, or religious preferences. Experience providing care to socially and medically complex patients effectively in a variety of non-traditional settings. Knowledge of formal and informal community agencies and resources. Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services. Bilingual skills desired. Experience in serving the Medicaid Populations in urban or rural environments with familiarity of local formal and informal resource networks preferred. Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPPA, C.L.A.S.
